The following is a selection of question types that are often used in online surveys. Surveys and questions can be customized in a number of ways. For example, they can be accessible by e-mail invitation only, specific questions on the survey can be required, or questions that respondents see can be dependent on previous answers.

1. What is your first name?

This is a fill-in-the-blank question that is required. If it is skipped, an error message will appear when you click “Next”.

 


2. How did you hear about the Puget Sound Center? Check all that apply.
This is a check-all question, meaning that participants can select more than one answer.

Through TechREACH
Through the National Girls Collaborative
Through the Peer Coaching Program
Through the technology courses or computer lab
Through the Northwest Girls Coalition

3. How many online surveys have you taken in your lifetime?
This is a choose-one question that is also required. Your answer on this question determines the next question you will be asked. This is known as branching.
